  How to report a claim

There are three ways you may report a claim 24 hours a day, 7 days a week.

Please be prepared to furnish as much of the following information as possible:

  • Your policy number
  • Date, time, and location of the loss/accident
  • Details of the loss/accident
  • Name, address, and phone number of any involved parties
  • If applicable, name of law enforcement agency or fire department, along with the incident number

Report a claim by phone
Call 800-423-7675 option 1 to report a claim.

Report claim by email
Email your claim to ESSReportALoss@nationwide.com.

Report a new loss online.
